The Alliance for Healthier Communities has developed an evidence-informed Model of Health and Wellbeing (PDF) to guide delivery of primary health care. The model defines health in the same way as the World Health Organization:

The Alliance for Healthier Communities’ Health Equity Charter (PDF) is a commitment to action by the Alliance and its member agencies to recognize and confront barriers to equitable health.
