Important Notice to Clients About Coronavirus (COVID-19)
Our Primary Health Care team consists of an interdisciplinary group of dedicated professionals that includes physicians, nurse practitioners, nurses, a dietitian, respiratory therapist, chiropodist and foot care nurses.
Our team works closely with other teams at Carlington CHC and with community partners to support our clients. We work in partnership with our clients to help them achieve optimal health and well-being with the goal of fostering a healthier community through the delivery of comprehensive primary health care services to our diverse patient population, always conveying respect, compassion and concern.
Services are provided in both English and French. We will make every attempt to service clients in their first language. When we are unable to do so, we have access to and can provide professionally trained Interpreters who speak many languages.
Medical phone lines are open on Monday, Tuesday, Wednesday and Friday from 9:00 a.m. to 4:00 p.m. and on Thursday from 1:00 p.m. to 4:00 p.m.

Please bring all health insurance cards or certificates with you to all appointments. This includes OHIP and out of province cards and Interim Federal Health certificates.
We also ask that you bring your medication to your appointment, including any over-the-counter medications, to ensure your medication profile is current.
New clients seeking medical services must complete a registration form and medical questionnaire and meet with the Intake Counsellor. Forms are available from main reception. Intake is open on a first come walk-in basis Monday to Friday from 1:00 to 4:00 p.m. or by appointment at 613-722-4000 ext. 260.
