Assertive Community Treatment

Through collaborative partnerships, the Carlington Assertive Community Treatment supports individuals in their recovery process. We serve individuals with serious, complex, long-term, mental health issues. ACT service will be determined by a consultation with the potential client, ACT staff and a central referring source. Our team of mental health professionals includes a psychiatrist, nurses, a social worker, mental health counsellor, a peer specialist and addictions counsellor, who provide intense, comprehensive, community-based recovery service to clients 24-hours/day, 365 days/year for an indefinite time.

Types of Services:

Eligibility:

Eligibility for ACT Team services are determined by a consultation with Central Intake, ACT staff and the potential client. Clients must have a diagnosis of a serious mental illness and must have a minimum of 50 days in hospital for mental health reasons in the past year. Clients must have significant functional impairments such as an inability to consistently perform the range

of activities of daily living required for basic adult functioning in the community.

24-Hour Crisis Support

By referral, our team of mental health professionals includes a psychiatrist, nurses, a social worker, mental health counsellors, a peer specialist and addictions counsellor. Counselling

Free and confidential counselling services with access to a Psychiatrist for registered clients by referral. Services address a wide variety of life challenges from a client-centered, strength-based perspective.

Family Support

We offer education and support to families to strengthen relationships and connections that are key to recovery and community reintegration. Involving loved ones in the care journey helps build understanding, resilience, and a strong support network, all of which contribute to successful rehabilitation and long-term well-being.

Community Integration

Our community integration services empower clients with daily living skills training, tailored classes, and group skill-sharing opportunities aimed at strengthening social relationships. We also provide housing assistance, self-advocacy support, and help connect clients to essential community services, fostering independence, growth, and sustained connection within the community for a better future.
