Our Primary Health Care team is an interdisciplinary group of dedicated professionals, including: Physicians, Nurse Practitioners, Nurses, Dietitian, Respiratory Therapist, Chiropodist and Foot Care Nurses.
We work closely with other Carlington CHC teams and community partners to support our clients and help them achieve optimal health and well-being. Together, we aim to foster a healthier community through comprehensive, respectful, and compassionate care.
Language Accessibility
Services are provided in English and French. When needed, we can arrange professionally trained interpreters who speak many languages to ensure clear communication and inclusive care.

Medical phone lines are open:
- Monday, Tuesday, Wednesday and Friday from 9:00 a.m. to 4:00 p.m.
- Thursday from 1:00 p.m. to 4:00 p.m.
📞 Appointment Booking
Online Booking System: We are pleased to offer a NEW online booking system to all registered patients who wish to book a regular appointment type with their family doctor or nurse practitioner. Please note that the online appointment system should not be completed if you require the following appointment types:
- An urgent appointment
- Â An appointment within three business days
- Prenatal appointment
- Well-baby appointment
- Well-child appointment
- Health check/ Physical appointment
- Diabetes appointment
Call for Appointment: To call to book an appointment availabl, please call our Main Reception at 613-722-2161.
Please bring all health insurance cards or certificates with you to all appointments. This includes OHIP and out of province cards and Interim Federal Health certificates. We also ask that you bring your medication to your appointment, including any over-the-counter medications, to ensure your medication profile is current.

Flu season is here — protect yourself and your loved ones. We are now accepting flu shot appointments for registered clients. To book your flu shot, please call our main reception at 613-722-2161, or booking online. Stay healthy and help keep our community safe this season!
